Using Your Rewards

As you earn money, it'll be added to your new   Visa® Prepaid Card. You'll receive your card by mail after you complete your first screening or care visit.

Use your  Visa Prepaid Card to help you pay for a variety of products and services*:

  • Purchases at Walmart and Sams Club
  • Utilities — like your gas, electric and water bill
  • Telecommunications — like internet, cell and home phone bill
  • Transportation — like the bus, train, taxi or ride-share
  • Education — like expenses for K-12, college and trade schools

Remember to keep your Visa® Prepaid Card after you use it. As you earn more money, it'll automatically be added to your card.

Your rewards are good for a full year. Funds expire 90 days after termination of insurance coverage or 365 days after date rewards were earned, whichever comes first.

Requirements for Earning Rewards

All rewards are limited to one per calendar year.

Vaccine must be received during flu season (September - March).

Rewards can be earned for completing an HbA1c test, kidney screening (urine protein test), and retinal (eye) screening.

The Annual Wellness Visit is a yearly appointment with your primary care provider (PCP) to create or update a personalized prevention plan. It is not a head-to-toe physical.

Complete a follow-up visit with a mental health practitioner after being discharged from an acute inpatient hospitalization that included a mental health diagnosis. The follow-up must occur within 30 days of being discharged.

Your reward will depend upon the type of screening your doctor recommends. Only one test per year will be rewarded. The various screening types and rewards available are as follows:

  • Colonoscopy
  • Flexible Sigmoidoscopy
  • Fecal Occult Blood Test (FOBT)
  • Fecal Immunochemical Test (FIT)
  • Stool DNA test

The My Health Pays® Visa Prepaid Card is issued by The Bancorp Bank pursuant to a license from Visa U.S.A. Inc. The Bancorp Bank; Member FDIC.

*This card is limited to qualifying products and services as listed above. Eligible items up to the amount of your card balance will be covered. Any remaining balance will remain on your card. You can use it for future purchases. This card may not be used everwhere Visa debit cards are accepted. See Cardholder Agreement for complete usage restrictions.
